Consumer Complaint: Collections Accou nts Skim disc over after conducting an investigation in conclusion this account is a result of identity theft 1. This account is from the same original creditor as a medical billing note : account is been reported with XXXX collector as appear that both collector own same debt Syndicated office systems and Central Financial Control claiming total control of the accoun t 2. An other thing to note as is a violation HIPAA regulations in the United States prevent any medical documentation to be shared. 3. Whe n I requested to credit bureau to conduct an investigation with requiring to summit documentation as part of verification medical debt collection agency responded as verified making their respond to be not valid proof as well the credit reporting agency did not follow proper investigation as they did not remove this account from my file is a violation on the credit reporting agency in this case was conducted by XXXX 4. So they could not provide the needed information to get the debt validate if they have the right to collect this medical bill they would have to have writing consent from the person HIPAA regulations as is required by law under the HIPAA 5. Proof of sign a HIPAA fo rm when we go to the doctor or a hospital for services render to give them the right to view any medical record, and not having so, they could not have any detail information therefore making this another violation Because of what was explain above I demand for this to be delete immediately from my credit file from all XXXX credit reporting agencies and provide to me in writing that this request was complete Also if there is any documentation that they have that could help to the investigation have documents send to me by mail to the mailing address they have on records of me so that I can present them to the investigation team It is clear that this alleged account and the services provided my personal identification was used as a result of identity theft
