My letter specifically requested for validation of the debt by providing me with something that shows my signature giving my permission to the medical facility/provider to release my information to a third party collection agency and received an itemized bill but nothing containing my signature. This is direct violation of many HIPAA laws and I am requesting to have this debt removed from my report because PMAB failed to provide validation of this debt. Also, the debt was previously removed from my XXXX XXXX XXXX report due to failure of this company providing validation of the debt.
