Consumer Complaint: On XXXX at XXXX am XXXX XXXX received certified letter requesting proof of original application, contract, any and all statements related to this debt, signed receipts, canceled checks, and or documentation containing my original signature obligating me to a debt that the claim I owe. Since, I have only received a screenshot of a piece of paper with my name, an account number, as means of validation. As per the Fair Debt Collection Act, section 809 ( b ), XXXX XXXX is in violation by continuing collection efforts without providing the requested documentation from the consumer as well as the original creditors information. I am also aware that the Health Insurance Portability Act or ( HIPPA ) regulations in the United States prevent any medical documentation to be shared. In addition XXXX XXXX received a notarized affidavit requesting original documentation containing my signature regarding this debt. Since XXXX XXXX has failed to provide the requested documentation, I expect this debt to be removed from the all three of the major consumer reporting agencies immediately.
