Consumer Complaint: On XX/XX/XXXX at XXXX, I called the Phoenix Financial Services collections agency to get further information about an account listed on my credit report.The original creditor is XXXX XXXX XXXX Hospital and I do not recall the encounter with them. The representative at the collections agency told me that the bill is from XX/XX/XXXX. She said that I had been seen as a patient at XXXX XXXX XXXX Hospital and was diagnosed with XXXX. I explained to her that I would need a copy of the original signed documents because I did not recall being seen at this hospital at this time, nor do I recall having ever been diagnosed with XXXX. She told me that she would send me what she needs to legally send me since I was making this request. I told, specifically, that I am specifically requesting a copy of the signed original agreement between myself and the hospital, so that I could indeed confirm that I signed as a patient on the date in question. The representative told me that her office had no such document. I concluded the call at that time. This call was recorded.
